The 2027 offer date is nearly two weeks later than 2026's because the whole Round 1 interview window runs to 2 April 2027. Posts start between August and December 2027.
How is the CST ranking decided?
The CST ranking is decided by the selection score, which in the published 2026 weighting is MSRA 10%, portfolio station 45% and management and clinical station 45%, out of 100. NHS England's offers page says appointments are made "in rank order, based on selection scores".
The selection score is the single number out of 100 that combines your scaled MSRA and your two interview stations; it is the only thing that fixes your position in the queue for offers.
For 2027, prioritisation under the Medical Training (Prioritisation) Act applies from shortlisting through to offers, so the queue is ordered within prioritised groups before score. The 2027 criteria are not yet published, and the coresurgeryinterview guide pages will carry them when they are.
How do preferences decide which CST post you are offered?
Preferences decide which post you get once your rank is reached, and nothing before that. NHS England's preferencing page states: "The order offers will be made in will be determined solely on the selection score and permissions to work in the UK, not preferences made".
When it is your turn, "you will be considered for all your preferences before any candidates ranked below you". So you receive your highest-ranked preference that still has a vacancy, and everyone below you is considered only after you.
That said, ranking an unpopular region high to "play safe" gives no advantage. The same page says preferences "only come into play once it is your turn to be made an offer", so put them in the order you actually want them.
Can CST preferences be changed after interview?
Yes, while a preference window is open. Windows operate "in between offer cycles, up until the upgrade deadline for the recruitment round", so preferences can be reordered after interview and between offer rounds, but not after 4pm on 14 April 2027.

How do CST upgrades work in 2027?
CST upgrades move you automatically to a higher-ranked preference if a post there becomes vacant before the upgrade deadline of 4pm on Wednesday 14 April 2027. NHS England's offers page says you are moved "to a higher-ranked preference if it becomes available", with no advance notice, and that "upgrades will not continue beyond the stated upgrade deadline".
An upgrade is an automatic reallocation to a post you ranked higher than the one you hold, triggered when a candidate above you declines or is upgraded elsewhere. After the deadline, vacant posts go to new candidates rather than to those already holding offers.
However, opting in is a commitment. Because it happens without notice, only rank above your current post the places you would take on the day.
What is the hierarchical deadline for CST 2027?
The hierarchical deadline for CST 2027 is 4pm on Thursday 15 April 2027, one day after the upgrade deadline, and it closes the last movement between an accepted CST post and a higher preference. It is the final fixed date in the Round 1 timeline.
The offers page describes hierarchical upgrades as the one exception to the rule that accepting an offer ends all others: an accepted candidate can still move up within their preference list until this point.
What is a competitive rank for CST in 2027?
NHS England does not publish the rank at which CST offers stop, so no number is given here. What is published is the scale of the round: in 2025 there were 630 posts for 5,399 applications, a ratio of 8.57 to 1, and in 2026 the interview invitation went, according to our fact table, to the top-scoring 1,200 applicants at the MSRA.
That said, the arithmetic of the published figures sets a rough shape: if around 1,200 are interviewed for around 600 posts, roughly half of interviewed candidates receive an offer. When do you get CST interview feedback?
CST feedback, including your scores and your ranking, is due "no later than 7 days after the national offer deadline", according to NHS England's offers, feedback and upgrades page. Feedback follows longlisting, shortlisting and interview stages.
The feedback is the most useful document of the year for anyone reapplying, because it shows which of the two stations, or the MSRA, cost the rank. Keep it.
What should you do if you receive no CST offer in 2027?
If you receive no CST offer in 2027, the plan is to read the feedback, check whether you remain eligible under the 18-month surgical experience cap in the person specification, and rebuild the weakest scored component before applications reopen in October 2027. Nothing in the 2027 process carries over: the MSRA is resat, the evidence is re-uploaded and the interview is repeated.
- Read the scores. A weak MSRA rank means January is the priority; a weak station means spoken practice is.
- Check eligibility. The 2027 person specification allows 18 months or less of surgical experience, whole-time equivalent, excluding foundation placements, at the post start date. A further year in a surgical post may take you over it.
- Raise one portfolio letter. A closed audit loop, a delivered teaching programme of four sessions, or a five-day taster week each moves a domain grade under the 2025/26 descriptors.
